Most of the time, it is either purposefully based on the song, or it is accidentally based on the song because it was in the back of the composer's mind while they were writing. When Hans Zimmer was doing the score for The Dark Knight, he had been listening to a lot Beethoven (specifically "Moonlight Sonata") and he admits that the main theme of the Dark Knight borrows very heavily from that.
